21st Century Home Health Services (21HHS) in the Bay Area is seeking a Certified Home Health Aide (CHHA) to support patients in the Milpitas and surrounding South Bay communities. You will work under an RN Case Manager to deliver in-home care according to the Plan of Care and help patients maintain independence.

We emphasize teamwork, growth, and compassionate service, with a focus on high-quality outcomes and respectful engagement with patients and families.
