Location: Los Angeles County including Whittier, Bellflower, Norwalk, Downey & surrounding areas.

Key Responsibilities

• Provide personal care assistance including bathing, grooming, dressing, and toileting.
• Support mobility and rehab activities, including transfers, ambulation, and exercises.
• Monitor and record vital signs during home visits.
• Assist with meal preparation and feeding based on patient care plans.
• Offer medication reminders (no administration required).
• Perform light housekeeping to maintain a safe and clean home environment.
• Observe and report changes in patient condition to the clinical team.
• Follow individualized home health care plans under RN or therapist supervision.
