Responsibilities

• Provide personal care assistance, including bathing, grooming, and dressing.

• Monitor and record vital signs during home visits.
• Assist with meal preparation and feeding based on patient care plans.
• Offer medication reminders without administration.
• Perform light housekeeping for a safe home environment.
• Observe and report changes in patient condition.
• Follow individualized care plans under RN or therapist supervision.
