Job summary

AbsoluteCare seeks a Community Health Worker to connect members with resources and guide enrollment in services. You will meet people wherever they are—at home, in the community, or at a clinic—and coordinate with the care team to address social determinants of health.

Responsibilities include assessing needs, creating care plans, linking to resources, and maintaining supportive, trust-based relationships while meeting program metrics.

Qualifications

• High school diploma or equivalent required.
• CPR/BLS certification required.
• Certified as a Community Health Worker required prior to employment.

Responsibilities

• Represent AbsoluteCare and explain services to members.
• Conduct in-person non-clinical assessments of member needs and barriers to care.
• Create and administer individualized care plans with clinical input.
• Connect members with community resources including housing, transportation, food, and more.
• Build trust and rapport with members and their support networks to drive engagement.
• Manage caseloads, enroll members in services, and follow through on care plans.
