Job summary

Advanced Center for Nursing and Rehabilitation in New Haven, CT seeks a full-time Central Supply and Transportation Coordinator to manage medical supplies and coordinate resident appointments and transportation. You will support nursing operations, ensuring timely access to supplies and safe, on-time transportation for residents.

Qualifications

• High school diploma or equivalent preferred.
• Experience in central supply, inventory management, medical office scheduling, transportation coordination, or a skilled nursing facility preferred.
• Strong organizational, communication, and time-management skills.
• Basic computer skills, including email, spreadsheets, and scheduling systems.
• Knowledge of medical terminology and healthcare documentation preferred.
• Ability to work collaboratively with nursing staff and other departments.
• Commitment to resident dignity, safety, and confidentiality.

Responsibilities

• Maintain inventory of medical, nursing, and facility supplies.
• Monitor stock levels and reorder supplies as needed to prevent shortages.
• Receive, inspect, organize, and distribute incoming supplies to designated departments.
• Maintain a clean, orderly, and secure central supply area.
• Track supply usage, deliveries, and inventory records.
• Communicate with vendors and department managers regarding orders, backorders, and delivery concerns.
• Monitor expiration dates and rotate stock as appropriate.
• Assist with supply cost control and minimizing waste.
• Follow facility policies for storage, infection prevention, and safety.
• Arrange transportation through facility vehicles, contracted providers, ambulance services, or other approved resources.
• Coordinate pickup and return times to support timely arrival and safe return to the facility.
• Verify transportation arrangements, including wheelchair accessibility and other identified mobility needs.
• Communicate transportation requirements to nursing staff and transportation providers.
• Notify staff of delays, cancellations, or transportation changes.
• Maintain transportation logs and related records according to facility policy.
• Assist with resolving scheduling conflicts and identifying alternative transportation arrangements.
• Maintain resident privacy and confidentiality in accordance with HIPAA and facility policies.
• Communicate professionally with residents, families, staff, vendors, and outside providers.
• Prioritize competing responsibilities and meet established deadlines.
• Attend required meetings and training sessions.
• Perform other related duties as assigned by facility administration.
