Qualifications

• Active RN license or LPN/LVN in good standing.
• Minimum of 2 years of skilled nursing facility MDS experience.
• Proficiency with electronic medical records and MDS software systems.
• Strong organizational and time-management skills.
• Ability to work independently in a centralized corporate environment.

Responsibilities

• Complete all required discharge MDS assessments accurately and within CMS timelines.
• Review medical records to ensure assessment accuracy and compliance.
• Coordinate with facility clinical teams to obtain missing documentation for MDS completion.
• Validate discharge coding, discharge status, and discharge tracking information according to CMS requirements.
• Monitor discharge activity reports and assessment due dates across assigned facilities.
• Monitor discharge MDS coding practices to support Quality Measure outcomes.
• Analyze quality measure trends and identify opportunities for coding accuracy and documentation improvement.
• Assist with quality measure validation reviews and performance improvement projects.
• Data accuracy and quality assurance.
