  • Monitor discharge activity reports and assessment due dates across assigned facilities.
  • Monitor discharge MDS coding practices to support Quality Measure outcomes.
  • Analyze quality measure trends and identify opportunities for coding accuracy and documentation improvement.
  • Assist with quality measure validation reviews and performance improvement projects.
  • Data accuracy and quality assurance.

Corporate MDS Specialist

The Corporate MDS Specialist is responsible for the timely and accurate completion of required discharge MDS assessments for residents discharged from skilled nursing facilities within the organization. This position is based in the management office and serves as centralized MDS resource supporting multiple facilities. The position is not assigned to or based at an individual nursing facility.

Essential Responsibilities
  • Complete all required discharge MDS assessments accurately and within regulatory timelines.
  • Review medical records and supporting clinical documentation to ensure assessment accuracy and compliance.
  • Coordinate with facility clinical teams to obtain missing or clarifying documentation necessary for MDS completion.
  • Validate discharge coding, discharge status, and discharge tracking information according to CMS requirements.
  • Monitor discharge activity reports and assessment due dates across assigned facilities.
  • Monitor discharge MDS coding practices to support optimal Quality Measure (QM) outcomes.
  • Analyze quality measure trends and identify opportunities for coding accuracy and documentation improvement.
  • Assist with quality measure validation reviews and performance improvement projects.
  • Data accuracy and quality assurance.
Compliance & Quality Assurance
  • Ensure compliance with CMS, Medicare, Medicaid, and state-specific documentation requirements.
  • Perform quality reviews of submitted discharge assessments to minimize errors and maximize data integrity.
  • Identify documentation concerns, coding opportunities, and compliance risks related to discharge assessments.
  • Assist with preparation and response to MDS-related audits and regulatory reviews.
  • Maintain current knowledge of RAI Manual updates, CMS guidance, and industry best practices.
Management Support
  • Serve as a centralized MDS resource to facility MDS Coordinators, DONs, Administrators, and regional clinical leaders.
  • Collaborate with Revenue Cycle, Clinical Reimbursement, and Operations teams to support accurate reimbursement and quality reporting.
  • Participate in educational initiatives and training related to discharge MDS processes.
  • Support corporate quality improvement initiatives related to MDS accuracy and compliance.
Required Qualifications
  • Active RN license or LPN/LVN license in good standing.
  • Minimum of 2 years of skilled nursing facility MDS experience.
  • Strong working knowledge of CMS MDS 3.0 and RAI Manual requirements.
  • Experience with Medicare PPS, PDPM, and discharge assessment completion.
  • Proficiency with electronic medical records and MDS software systems.
  • Excellent organizational, communication, and time-management skills.
  • Ability to work independently in a centralized corporate environment while managing multiple facility assignments.
Preferred Qualifications
  • RAC-CT or other MDS certification.
  • Multi-facility or consulting MDS experience.
  • Corporate, regional, or centralized clinical reimbursement experience.
