Job description

The Clinical Director will directly supervise and train primary care providers (PCPs) in the assigned center, ensuring maximization of core model execution, including clinical quality, efficiency, outcomes, and clinician/patient satisfaction.

Responsibilities
  • Provide independent care for patients with acute and chronic illnesses in older adults, maintaining full accountability for patient outcomes.
  • Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with complex needs, documenting all care appropriately.
  • Leverage a care team that may include a medical assistant, care facilitator, and care coordinator for office‑visiting patients.
  • Coordinate with transitional care teams, case managers, and other resources for patients unable to visit the office, including those in hospitals, SNFs, LTCs, or homebound.
  • Lead weekly Super Huddle (SH) and Transforming Care Meeting (TCM) sessions.
  • Support the Center Manager or Center General Manager when the clinical leader is unavailable, monitoring daily census and championing joint accountability for outcomes.
  • Assume clinical leadership duties as needed, including recruiting patients, interviewing additional providers, and covering for absent providers.
  • Mentor, coach, and manage PCPs to achieve outstanding clinical outcomes, sampling their daily huddles and rounding with them.
  • Partner with Center Operations Director or Market General Manager to improve center financial performance and increase membership.
  • Perform additional duties as assigned by management.
Qualifications
  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
  • Current, active MD licensure in the state of employment.
  • Board eligibility in the terminal specialty (Internal Medicine, Family Medicine, Geriatrics) required; board certification preferred.
  • Minimum of one year of clinical experience in geriatric, adult, or family practice settings.
  • Completion of ChenMed training, including Masterful Conversations and meeting facilitation.
  • Current DEA number for Schedule II–V controlled substances.
  • Basic Life Support (BLS) certification within 90 days of employment.
  • Fluency in English, with additional language fluency required for certain centers.
  • Professional proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
  • Willingness to travel locally, regionally, or nationally for training and recruitment when required.
  • Independently exercise judgment in clinical and administrative matters.
Pay Range

$238,832 – $341,189 (salary)
