ESSENTIAL JOB DUTIES / RESPONSIBILITIES
- Independently provide care for patients with acute and chronic illnesses, especially in older adults.
- Take full accountability for patient care outcomes, seeking specialist consultation as needed, and remain responsible for detailed patient care.
- Engage hospitalists for our patients in the hospital, regardless of hospitalist affiliation.
- Assess, diagnose, treat, manage, educate, promote health, coordinate, and document care for patients with acute and complex chronic needs.
- Lead a care team consisting of a care promoter (medical assistant), a care facilitator, and a care coordinator for patients who can attend the office.
- For patients who cannot attend the office – in hospital, skilled nursing facility, long‑term care, or homebound – engage the transitional care team, case managers, physicians and other available resources.
- Lead Super Huddle and Transforming Care Meeting weekly, and support the Center Manager, Center General Manager, or market clinical leader as needed.
- Fill in for center clinical leadership needs and monitor daily center census as part of joint center accountability.
- Participate in recruiting patients for the center and recruiting, interviewing additional providers.
- Manage, mentor, and coach PCPs in the assigned center to deliver clinical outcomes and sample other PCPs’ daily huddles.
- Lead rounding with PCPs, partnering with the Center Operations Director or Market General Manager to drive improvement of center financial performance and member growth.
- Perform other duties as assigned or modified by management.
KNOWLEDGE, SKILLS, AND ABILITIES
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other company applications.
- Willingness and ability to travel locally, nationally for training, and regionally or nationally for recruiting or training.
- Fluent in English, both verbal and written; other languages may be required depending on location.
- Requires independent judgment.
- Flexible to work evenings, weekends, and holidays as needed.
EDUCATION AND EXPERIENCE CRITERIA
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Current, active MD licensure in the state of employment.
- Minimum one year of clinical experience in geriatric, adult or family practice setting preferred.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred; board eligibility required. PCP will maintain board certification through MOC, CME, or retaking exams as required.
- Current DEA number for schedule II–V controlled substances.
- Basic Life Support (BLS) certification from AMA or American Red Cross required within first 90 days of employment.
PAY RANGE
238,832 to 341,189. The posted pay range represents the base hourly rate or base annual full‑time salary. Final compensation will depend on experience, education, location, and other relevant factors. The position may also be eligible for bonuses or commissions.
