Job Overview
The Associate Center Clinical Director directly supervises and trains primary care providers (PCPs) in an assigned center, managing overall clinical outcomes, quality, efficiency, and patient and clinician satisfaction. A portion of the role’s time is dedicated to direct patient care as a PCP, while the remainder focuses on leadership responsibilities such as PCP performance, engagement, and building a strong clinical-operations synergy and culture.
Responsibilities
- Independently provide care for older adult patients with acute and chronic illnesses, taking full accountability for patient care and outcomes.
- Seek specialist consultation when needed and remain responsible for the detailed care of the patient.
- Engage with hospitalists for patients admitted to hospitals, regardless of hospitalist affiliation.
- Perform assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
- Lead the care team, including medical assistant, care facilitator, and care coordinator, for office-eligible patients.
- For patients unable to come to the office—such as in hospital, SNF, LTC, or homebound—engage the transitional care team and other resources like case managers and functional care physicians.
- Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, and support center leaders when unavailable.
- Fill in as needed for center clinical leadership duties, monitor daily center census, and assist with joint center accountability for outcomes.
- Actively recruit patients for the center and interview additional providers for the company.
- Manage, mentor, and coach PCPs in the assigned center, including sampling daily huddles and practicing leadership rounding.
- Partner with Center Operations Director/Market General Manager to drive continued improvement of center financial performance and increase center membership.
- Perform other duties as assigned and modified at the manager’s discretion.
Knowledge, Skills, and Abilities
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other company applications.
- Ability and willingness to travel locally, nationally for initial training (2 weeks), and regionally/nationally for recruiting or training purposes.
- Fluency in English (verbal and written); additional language fluency may be required at specific centers.
- Use of independent judgment in clinical and leadership decisions.
Education and Experience Criteria
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current, active MD licensure in the state of employment.
- Minimum of 1‑year clinical experience in geriatric, adult, or family practice setting preferred.
- Completion of Chen Medical training, including Masterful Conversations and meeting facilitation.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty preferred; board eligibility required.
- Board‑certified PCPs must maintain certification through necessary MOC, CME, and/or retaking board exams as required.
- Current DEA number for schedule II‑V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within 90 days of employment.
Pay
Pay Range: $238,832 – $341,189. Final compensation will depend on experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
