TheCommunity-Based Care Coordinator (CCS Service Facilitator) plays a vital role in providingtrauma-informed, empowerment-based care coordination to individuals with high-level mental health and/or substance use needs. This role focuses on deliveringpsycho-social rehabilitative services in diverse settings, including homes, schools, and community spaces, to help clients achieve recovery and self-determination.
Under the guidance of theMental Health Services Supervisor, the Service Facilitator will conductmental health assessments, developpersonalized recovery treatment plans, and facilitatepsycho-social rehabilitation to reduce the negative impacts of mental health and substance use disorders. The ideal candidate is passionate aboutsocial justice, cultural humility, and trauma-informed care, with a strong commitment to serving marginalized communities, including survivors of sexual trauma, clients of color, LGBTQIA+ individuals, and other underserved populations.
This role requiresempathy, adaptability, and a deep understanding of privilege, oppression, and systemic barriers affecting client needs. The Service Facilitator will collaborate closely with a multidisciplinary team, maintain accurate records, and contribute to continuous improvement in service delivery.
Key Responsibilities
Client-Centered Care & Support
- Providepsycho-social rehabilitative services that promoteself-determination, empowerment, and recovery for clients of all ages.
- Conductmental health assessments, includingsuicidal and homicide risk assessments, and developpersonalized recovery treatment plans in collaboration with clients.
- Facilitateindividual and group psychoeducation, skill development, and trauma-informed support tailored to the unique needs of marginalized survivors, including considerations forrace, ethnicity, gender, gender identity, religion, age, ability, and other intersecting factors.
- Ensureculturally sensitive and trauma-informed service delivery, with a focus onmarginalized survivors of sexual trauma, clients of color, LGBTQIA+ individuals, and other underserved communities.
- Maintainaccurate and up-to-date client records, including recovery plans, case notes, and master client lists in compliance with agency policies andRCC Helpline training requirements.
Collaboration & Professional Development
- Participate inindividual and group supervision, weekly case reviews, and team meetings to ensure alignment with best practices and continuous improvement.
- Provideregular reports to the Mental Health Services Supervisor onservice gaps, survivor needs, areas for improvement, and professional development opportunities.
- Stay current withapplicable statutes, administrative codes, and agency policies to ensure compliance and high-quality service delivery.
- Attendregular staff meetings, in-services, and professional development opportunities to enhance skills intrauma-informed care, cultural humility, and empowerment-based models.
Administrative & Operational Support
- Keep thecalendar up to date in Outlook and ensure timely documentation of services provided.
- EnterCCS case notes into the county moduleweekly and maintain organized records for audits and client follow-ups.
- Assist inoffice operations and contribute to acollaborative, supportive work environment.
- Perform other duties as assigned to support themission and goals of the organization.
Qualifications & Skills
Required Qualifications
- Education: Bachelor’s or master’s degree inSocial Work, Counseling, Psychology, or a related field.
- Experience: Demonstrated experience providingtrauma-informed, empowerment-based services to individuals with mental health and/or substance use disorders, with astrong preference for experience serving survivors of sexual trauma, clients of color, LGBTQIA+ individuals, and other marginalized communities.
- Licensing & Training: Commitment to maintaininglicensing and training records as required by state regulations.
- Cultural Competency: Clear understanding ofprivilege, oppression, and systemic barriers affecting client needs, with a commitment tocultural humility andtrauma-informed care.
- Communication: Excellentverbal and written communication skills, with the ability to document services clearly and professionally.
- Flexibility: Ability to workflexible hours, including evenings or weekends, as needed to meet client needs.
- Background Check: Satisfactorycriminal background check.
- Confidentiality: Commitment to maintainingclient confidentiality and adhering toethical standards in all interactions.
Preferred Qualifications
- Bilingual (Spanish/English) proficiency.
- Knowledge of Dane County community resources and local support systems.
- Personal or professional experience incommunities of color or other marginalized communities.
Work Environment & Physical Requirements
- Primary Work Setting: Office, classroom, or community-based environments.
- Physical Demands: Ability to lift and move up to20 pounds, performdaily movement (e.g., walking, standing), and usecomputers, office equipment, and telephones.
- Work Conditions: Frequent interruptions, low to moderate noise levels, and dynamic environments requiring adaptability.
