Qualifications

• Bachelor's degree in healthcare field required.
• 4+ years in healthcare ops, care management, QA, or clinical program oversight.
• Strong knowledge of CCM/PCM workflows and CMS documentation requirements.
• Proficient in data analysis, reporting validation, and performance tracking.
• Ability to identify process gaps and implement scalable improvements.
• Excellent communication and cross-functional collaboration skills.
• Experience with EMRs and reporting tools preferred.

Responsibilities

• Support the Senior Director in executing CCM/PCM program goals and quality metrics.
• Oversee daily operations to ensure documentation accuracy and productivity.
• Provide real-time support to coordinators for workflow and charting challenges.
• Ensure policy and workflow implementation across all facilities.
• Lead reporting-focused initiatives to improve usability and audit readiness.
• Prepare facility-facing reports for DON outreach and leadership review.
• Collaborate with Program Leads, QA, Compliance, and Data Analytics teams.
