Rutgers University Behavioral Health Care (UBHC) in Newark is seeking a Habilitation Counselor for the CBHP within UBHC. You will provide site and community based case management and supported employment services to individuals in outpatient care.

Key duties include participating in hotline calls, assessing case management needs, connecting clients to hub resources, advocating for resources, completing documentation, and supporting CBHP objectives.

Qualifications

• Bachelor’s Degree in a behavioral health or related field required.
• One year of related experience preferred; licensure may be required for Social Work degree.

Responsabilités

• Active participation in CBHP team hotline calls and ensure consumer-friendly handling.
• Assess and manage case management needs of youth and families; connect to hub and community resources.
• Advocate for appropriate resources based on assessed needs.
• Complete documentation and utilization reviews.
• Support CBHP program and UBHC objectives; complete internal paperwork as required.
