Rutgers University Behavioral Health Care (UBHC) seeks a Habilitation Counselor for the Early Intervention Support Services (EISS) in Somerset. The role provides recovery-oriented case management to adults 18+, aligning with UBHC policies and procedures.

The position offers full-time, on-site work with a focus on developing recovery plans, coordinating resources, and collaborating with the treatment team to support client independence.

Qualifications

• Bachelor's degree in mental health field plus one year of related experience.
• Active New Jersey certification/licensure for Social Work.
• Valid driver's license.
• Bilingual English/Spanish preferred.
• CADC preferred.

Responsibilities

• Collaborates with treatment team members in assessment of newly referred consumers.
• Develops multidisciplinary, person-centered treatment plans with clear timeframes.
• Advocates for resources for clients and families based on assessed needs.
• Acts as liaison to community resources to enhance independent living skills.
• Makes community resources known to team and consumers.
• Maintains respectful, professional environment for all clients and staff.
• Promotes effective working relationships within the EISS unit team.
