Rutgers University Behavioral Health Care (UBHC) in Newark, NJ, seeks a Habilitation Counselor for the Outpatient Treatment – Certified Community Behavioral Health Clinic (CCBHC) Case Management Department. The role focuses on site and community-based case management and supported employment services to UBHC clients.

Responsibilities include helping clients access medical, social, educational, and mental health services, providing psychoeducation, and maintaining clinical records, with emphasis

Qualifications

• Bachelor’s Degree in the mental health field or other appropriate human services related field plus one (1) year of relevant mental health experience.
• Active New Jersey certification/licensure as required by the State Board of Social Work if degree is in Social Work.
• Valid driver’s license.

Responsibilities

• Supports individuals to gain access to needed medical, social, education, mental health, addiction, co-occurring, and other services.
• Provides psychoeducation to individuals served, promoting wellness and recovery goals.
• Demonstrates the ability to engage and establish a working relationship with and provide services to individuals who are diagnosed with a serious and persistent mental illness and co-occurring disorders who present engagement challenges.
• Collaborates effectively with department members, external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care.
• Communicates observations and information about the consumer to the program supervisor and staff involved with services.
• Maintains appropriate clinical records according to time frames and quality guidelines.
• Actively participates in and contributes to smooth functioning and operations of unit.
