Jefferson Health is seeking a registered nurse to provide patient-centered care at our Philadelphia area facilities. You will collaborate with physicians, therapists, and support staff to develop and implement individualized care plans, while ensuring safety and adherence to best practices.

You will educate patients and families, assess needs, and participate in discharge planning and quality initiatives, contributing to a caring culture and coordinated care across teams.

Qualifications

• Graduate of an accredited nursing program with a specialized diploma.
• Demonstrated ability to partner with patients and families in care planning.
• Knowledge of diverse cultural backgrounds and respectful communication.
• Strong, effective written and verbal communication with patients, families and team members.
• Knowledge of clinical assessment, care planning, and nursing diagnoses.

Responsibilities

• Interacts with patients, families, and staff to deliver patient-centered care.
• Integrates evidence-based practice to improve quality and safety of care.
• Uses IT systems to document care, communicate, and monitor outcomes.
• Participates in committees and takes part in quality and safety initiatives.
• Provides education to patients and families about conditions, medications, and discharge plans.
