Responsibilities

• Greet and escort patients through the medical practice.
• Conduct nursing assessments and document in the electronic medical record.
• Collaborate with physicians and health team to implement orders.
• Apply holistic Patient Centered Medical Home approach to care.
• Document and implement clinical and technical aspects of care.
• Evaluate patient response to the plan of care using teach-back methods.
• Communicate openly with patients, physicians and health team members.
• Serve as liaison for the patient with outside agencies and providers.
• Respect patient privacy, dignity and confidentiality.
• Maintain positive relationships with internal and external customers.
• Adhere to health system policies, safety, and infection control guidelines.
