Job summary

Qualifications

• Experience in disability or social services preferred.
• Knowledge of person-centered care principles preferred.
• Strong communication and interpersonal skills required.
• Ability to pass a background check and meet licensing requirements.
• Valid driver's license with clear driving record and reliable transportation.
• CPR/First Aid certification required (will be provided if needed).
• MAT: Wyoming Department of Health Medication Assistance certification required (will provide if needed).

Responsibilities

• Provide direct support to individuals with disabilities.
• Implement individualized service plans with clients and care teams.
• Advocate for clients' rights, independence, and access to resources.
• Assist with daily living activities, personal care, and mobility support.
• Maintain accurate documentation and compliance records.
• Coordinate with healthcare providers, families, and community organizations.
• Monitor progress and adjust support strategies accordingly.
