Job summary

Mindlance in New York City is seeking a Social Services Coordinator for a 7+ weeks contract. Under supervision, you will provide social services to clients and assess eligibility for public assistance programs, with a focus on coordinating care for patients in the community.

The role emphasizes care coordination tasks, referrals to medical, mental health and housing services, post-discharge follow-up, and documentation.

Qualifications

• Provide social services to clients, assess eligibility for public assistance programs.
• Care coordination tasks, in-person or telephonic, with patient escort as needed.
• Schedule management to allow field work, engagement, follow-up, documentation, and admin tasks.

Responsibilities

• Under supervision, provides social services to clients/patients and determines eligibility for services.
• Coordinate field/in-person or telephonic care and escort of patients.
• Manage schedule to allow for field work, face-to-face engagement, timely follow-up, and documentation.
• Provide patients/family assistance with system navigation.
• Facilitate referrals to community-based services (medical, mental health, housing, etc.).
• Conduct post-discharge follow-up when indicated.
• Provide coverage for facility placements and discharge notices.
• Support Primary Social Worker with concrete tasks (NOK searches).

Job description

Duration: 7+ Weeks Contract with potential for extension based on performance and needs

Shift: Monday - Friday: 9:00 AM to 5:00 PM

Location: New York NY 10016

Duties:

• Under supervision, provides social services to clients/patients, such as; recipients of public assistance, food stamps and/or medical assistance, adults receiving or needing institutional care or protective services. Identifies, develops and implements social service plans for such families/individuals - determine the eligibility for these services and may perform counselling and investigatory activity related to the work.
• Specific field/in-person or telephonic care coordination tasks as needed, escort of patients in community
• Manage schedule to allow for adequate time for: field/travel, face to face patient engagement, completion of time sensitive follow up, documentation, and administrative tasks.
• Providing patients/family assistance with system navigation.
• Facilitate referrals to a variety of community-based services including, but not limited to: medical, mental health, and substance treatment; housing, transportation, home care, DME, HD, respite and other needs as identified.
• Conduct post discharge follow up for patients when indicated.
• Providing back up coverage for facility placements, and 24-hour discharge notice distribution
• Providing support to Primary SW with concrete tasks, including NOK searches

Skills:

• 3 years' experience.

Education:

• A Baccalaureate Degree from an accredited college or university.
