Job summary

Community Counseling Center, Cape Girardeau, MO, is seeking a Full Time, non-exempt position to deliver community support services through CPRC. The role involves coordinating service delivery, assessing client adjustment, linking with resources, and responding to crises to reduce hospitalization risk.

Typical workload is 20–25 clients with weekly visits to homes or residential centers. The role requires driving, a MO driver’s license, and willingness to travel as needed.

Qualifications

• Qualifying experience must include delivery of service to individuals with mental illness, substance abuse disorders, or developmental disabilities.
• Valid Driver's License - MO Class E; travel in own vehicle is required.

Responsibilities

• Provide community support services to CPRC clients.

Job description

Full Time: Non - Exempt

Work Hours: Monday - Friday 8:00am - 4:00pm, Some evenings maybe required as necessary dependent upon agency and client need.

Pay Range: $20.00 - $22.40 per hour

Note: Pay will vary dependent upon prior experience.

Qualifications

• Bachelor's degree or higher from accredited university; or
• Any four-year combination of higher education and qualifying experience; or
• Four years of qualifying experience.
• Valid Drivers License - Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents) and current auto insurance.
• Travel in own vehicle is required.

Job Summary

Responsible for the delivery of community support services for assigned clients within the Community Psychiatric Rehabilitation Center (CPRC). These activities include coordination of delivery of mental health services, assessment and monitoring of client adjustment, resource development and linkage with community resources, providing crisis response to reduce risk of hospitalization of clients in crisis. Typically, the position manages 20 - 25 clients, meeting with them weekly at client homes or residential centers.

Job Duties

• Provide community support services to children and adult clients enrolled in CPRC program.
• Maintain a caseload as assigned including in the community and during hospitalizations.
• Maintain direct services productivity in accordance with assigned caseloads.
• Maintain positive attitude toward agency and clientele.
• Maintain appropriate boundaries with clients and staff members.
• Provide training/education to clients in daily living skills.
• Monitor completion of treatment plan goals and objectives and maintain contact with treatment team in regard to progress and any concerns.
• Refer to appropriate resources within the community to assist clients in maintaining/improving their functionality and to develop a personal support system.
• Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
• Successfully complete DMH approved Wellness Coaching training through the Relias Learning platform.
• Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
