Job summary

Community Counseling Center (CCC) in Missouri is hiring to support clients through the Clustered Apartments Program. You will assist with community support services, coordinate mental health care, and collaborate with a multidisciplinary team to assess and treat clients.

You will train clients in daily living skills, help build support networks, and encourage recreational activity participation within their communities, alongside on-site inspections and case management.

Qualifications

• HS Diploma/HSE/GED required.
• Must hold MO Class E Driver's License (Class D for IL Residents) and prove continuous auto insurance.
• Meet CCC's Motor Vehicle Record standards and have reliable transportation.

Responsibilities

• Assist with the delivery of community support services for clients.
• Coordinate delivery of mental health services.
• Develop resources and linkage with the community; work as part of a multidisciplinary team.
• Provide training to clients in daily living skills.
• Facilitate development of personal support systems within the client's community.
• Assist clients in creating recreational activities.
• Perform bi-monthly apartment inspections and advise on household management.
• Maintain productivity standards and manage assigned caseloads.
• Document according to DMH, CARF and agency standards.

Shift: Sunday-Thursday 8a-430p, Fri/Sat off.

Base Pay: $16.42 - 18.82/hour

Minimum Required Qualifications

• HS Diploma/HSE/GED is required.
• Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC's Motor Vehicle Record standards policy and have reliable transportation.

Due to this position requiring staff transportation of clients in extenuating circumstances, this position also requires: reliable transportation, valid driver's license, and proof of continuous auto insurance.

The Program

Clustered Apartments allow for on-site monitoring and on-site interventions and treatment of individuals who require an intensive level of care and support, and who may require skills in the necessities of daily living. Clustered Apartments provide low income subsidized housing assistance to persons with disabilities, mental illnesses and/or poverty.

The Job

Responsible for assisting with the delivery of community support services for individuals receiving services through our Clustered Apartments Program; coordination of delivery of mental health services; resource development and linkage with community; functions as a professional staff member of a multi-disciplinary team providing comprehensive assessment and treatment services to clients.

Essential Functions

• Provide training to clients in daily living skills.
• Facilitate development and/or enhancement of personal support systems within the client's community.
• Assist clients in creating and participating in recreational activities as indicated or available.
• Perform bimonthly apartment inspections and provide input/skills training on household management.
• Maintain work productivity standards as established by supervisor and in accordance with assigned caseloads.
• Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
