Qualifications

• Bachelor degree in a related field is required.
• MSW preferred.

Responsibilities

• Complete Initial Service Plan Review & Case Conferences for clients/families and facilitate meetings.
• Coordinate services identified in the Initial Service Plan to support clients' goals.
• Perform home assessments for clients utilizing weekend passes.
• Report suspected abuse or neglect to the appropriate authorities.
• Oversee Counseling Specialists and ensure quality service delivery.
• Provide aftercare services to clients for 90 days post discharge.
• Maintain case records and ensure necessary documents are present.
• Attend and participate in QA/Program meetings.
• Travel up to 25% of the time.
