Job summary

SCO Family of Service in Jamaica, NY is seeking a supervisor to oversee all case work services. You will ensure completion of required paperwork and full compliance with City and State RHY regulations and standards.

The role involves coordinating Initial Service Plan reviews, home assessments, reporting abuse, training in DYCD Connect, supervising Counseling Specialists, and delivering aftercare to residents for 90 days post-discharge.

Qualifications

  • Bachelor degree in a related field is required.
  • MSW preferred.

Responsibilities

  • Complete Initial Service Plan Review & Case Conferences for clients/families & facilitate additional family meetings and/or program visits.
  • Coordinate services identified in the Initial Service Plan & subsequent reviews to assist clients in meeting their short & long-term goals.
  • Complete home assessments for clients who will be utilizing weekend passes.
  • Report any suspected abuse or neglect to Justice Center/State Central Register.
  • Become DYCD Connect trained & proficient.
  • Oversee Counseling Specialists regarding their specific responsibilities supporting case management services.
  • Provide aftercare services to clients for 90-days post discharge.
  • Maintain case records & ensure necessary documents are present on all current residents in their 90-day aftercare period.
  • Facilitate weekly force field/team meeting.
  • Conducts initial intake interviews of prospective residents in the absence of Intake Coordinator.
  • Work collaboratively with Program Coordinator, Counseling Specialists and Intake Coordinator to ensure quality of service delivery.
