Job summary

SCO Family of Services is seeking a Program Supervisor to oversee all case work services, ensure documentation compliance, and align with RHY regulations. The role involves coordinating across teams, conducting home assessments, and arranging aftercare for clients and families.

The ideal candidate holds a Bachelor's degree in a related field with MSW preferred, and demonstrates strong organization, independence, and cultural sensitivity while managing caseloads and contributing to QA/QI

Qualifications

Bachelor degree in a related field required.
MSW preferred.

Responsibilities

Complete Initial Service Plan Review & Case Conferences for clients/families and facilitate additional family meetings and program visits.
Coordinate services identified in the Initial Service Plan to meet short- and long-term goals.
Complete home assessments for clients using weekend passes.
Report suspected abuse or neglect to the appropriate authorities.
Oversee counseling specialists and support case management services.
Provide aftercare services to clients for 90 days post discharge.
Maintain case records and ensure required documents are present for current residents.
Participate in staff meetings and QA/QI program admin meetings.
