Job summary

Thresholds is seeking a Case Manager for its Youth Transitional Living program in Chicago to provide wraparound support to youth and young adults aged 17–24 facing mental health challenges.

You will coordinate treatment, develop individualized service plans, facilitate services with schools, medical providers, families, and support networks, and coach youth one-on-one to build skills for adulthood.

Qualifications

• Bachelor’s degree required
• Master’s degree preferred
• LSW/LPC/LCSW/LCPC licensure preferred
• Must have access to a personal vehicle and valid driver’s license
• Experience with youth or young adults preferred
• Ability to obtain liability car insurance

Responsibilities

• Develops and implements Thresholds Care Plans with the transition team and families
• Provides direct service including assessment, case management, group support, crisis intervention and transition coordination
• Participates in crisis intervention per Therapeutic Crisis Intervention standards and supervises physical management when necessary
• Attends court appearances, DCFS meetings, and related proceedings; maintains proper safeguarding and reporting
• Completes Medicaid compliant service notes and required reports in a timely fashion

Job description

Are you passionate about supporting young people as they transition to adulthood? Consider becoming a Case Manager with Thresholds' Youth Transitional Living program that provides wraparound support to youth and young adults with mental health challenges. The Case Manager coordinates treatment, develops and implements individualized service plans, facilitates services – like school, medical providers, family, etc. – and also assists in skill building through coaching and one-on-one work with young adults ages 17-24.

ESSENTIAL DUTIES & RESPONSIBILITIES

• Works with the transition team, including the member, their therapist, residential staff, guardian(s) and others to develop and implement a Thresholds Care Plan. Serves as the primary contact with the members’ support network to assist with the members’ goals for transitioning to adulthood.
• Provides direct service to members through assessment, case management, group counseling, job support, crisis intervention, transition coordination, and psycho-social rehabilitation services in the community and at Thresholds locations.
• Performs and participates in crisis intervention in accordance to Therapeutic Crisis intervention (TCI) and Thresholds standards. Models best practices for TCI to other staff. Performs and supervises physical management when necessary.
• Attends all court appearances, administrative case reviews, DCFS meetings, and Thresholds meetings as assigned. Assures that all Thresholds procedures regarding the handling of member’s funds are followed and that all safeguards, reports, and accounting meets the highest standards.
• Completes all assigned paperwork in a timely fashion. Produces Medicaid compliant service notes. Completes reports for Thresholds, DCFS, ICG, DMH, courts, and others regularly and when required.

MARGINAL DUTIES & RESPONSIBILITIES

• May be required to work flexible schedules and be available to work on weekends or evenings
• May be required to be on call.

EDUCATION

• Bachelor’s degree in Psychology, Social Work, or related field required.

Preferred:

• Master’s degree in Psychology, Social Work, Counseling, or related field.
• LSW, LPC, LCSW, LCPC

REQUIREMENTS

• Daily access to a personal vehicle
• Valid driver’s license
• Willingness to provide transportation for members of the program
• Must obtain and maintain liability car insurance of:
  • $100,000 per person bodily injury
  • $300,000 per accident
  • $100,000 proper damage
• Previous experience working with young adults strongly preferred.
• Takes DCFS CWEL training and passes the exam
