Job summary

Thresholds is seeking a Case Manager for the Youth Transitional Living program in Chicago, IL. You will coordinate treatment, develop individualized service plans, and facilitate services with school, medical providers, family, and other support networks for youth aged 17-24.

You will provide direct services including assessment, case management, group counseling, job support, and crisis intervention, while maintaining Thresholds standards and safeguarding requirements.

Qualifications

• Bachelors degree is required for the role.
• Masters degree preferred for advancement and licensure support.
• Active DCFS CWEL license is required/licensure preferred.

Responsibilities

• Develop and implement Thresholds Care Plans with the transition team.
• Provide direct services: assessment, case management, group counseling, job support, crisis intervention.
• Perform crisis intervention and supervise physical restraint when necessary.
• Attend court appearances and DCFS meetings; ensure proper safeguarding and reporting.
• Complete Medicaid compliant service notes and required reports.

Job description

Are you passionate about supporting young people as they transition to adulthood? Consider becoming a Case Manager with Thresholds' Youth Transitional Living program that provides wraparound support to youth and young adults with mental health challenges. The Case Manager coordinates treatment, develops and implements individualized service plans, facilitates services - like school, medical providers, family, etc. - and also assists in skill building through coaching and one-on-one work with young adults ages 17-24.

Essential Duties & Responsibilities

• Works with the transition team, including the member, their therapist, residential staff, guardian(s) and others to develop and implement a Thresholds Care Plan. Serves as the primary contact with the members' support network to assist with the members' goals for transitioning to adulthood.
• Provides direct service to members through assessment, case management, group counseling, job support, crisis intervention, transition coordination, and psycho-social rehabilitation services in the community and at Thresholds locations.
• Performs and participates in crisis intervention in accordance to Therapeutic Crisis intervention (TCI) and Thresholds standards. Models best practices for TCI to other staff. Performs and supervises physical management when necessary.
• Attends all court appearances, administrative case reviews, DCFS meetings, and Thresholds meetings as assigned. Assures that all Thresholds procedures regarding the handling of members' funds are followed and that all safeguards, reports, and accounting meets the highest standards.
• Completes all assigned paperwork in a timely fashion. Produces Medicaid compliant service notes. Completes reports for Thresholds, DCFS, ICG, DMH, courts, and others regularly and when required.

Marginal Duties & Responsibilities

• May be required to work flexible schedules and be available to work on weekends or evenings
• May be required to be on call.

EDUCATION

• Bachelors degree in Psychology, Social Work, or related field required.

Preferred:

• Masters degree in Psychology, Social Work, Counseling, or related field.
• LSW, LPC, LCSW, LCPC

Requirements

• Daily access to a personal vehicle
• Valid driver's license
• Willingness to provide transportation for members of the program
• Must obtain and maintain liability car insurance of:
  • $100,000 per person bodily injury
  • $300,000 per accident
  • $100,000 proper damage
• Previous experience working with young adults strongly preferred.
• Active DCFS CWEL License
