Qualified candidates typically have a high school diploma or GED, with preferred CRSS and a related bachelor’s degree; bachelor’s + 2+ years or 5+ years substituted experience are acceptable pathways.

  • Trained in Basic First Aid and CPR.

Responsibilities

  • Provide direct service to members in residential setting and assess needs.
  • Teach independent and community living skills (housekeeping, shopping, meal prep, hygiene).
  • Maintain a safe, clean residence and report maintenance issues.
  • Support member self-advocacy and participation in planning and treatment.
  • Identify relapse risks and develop relapse prevention plans.

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Provides direct service to members in a residential setting, assesses service needs, and makes recommendations for treatment.
  • Teaches members independent and community living skills, such as housekeeping, shopping, meal preparation, and personal hygiene as well as the development of functional, interpersonal, and coping skills.
  • Responsible for creating a safe, clean, and inviting atmosphere for members. Keeps the residence neat and clean at all times. Advises supervisor, team leader, or other designee of any maintenance problems and issues.
  • Supports and promotes member self-advocacy and participation in planning, decision-making, and treatment.
  • Identifies risk factors related to relapse and develops relapse prevention plans and strategies.
  • Must have good documentation skills to write notes in Smartcare.
  • Monitors and documents self-administration of medication in keeping with Thresholds' standards.
  • Assures that all Thresholds records are organized. Follows all appropriate safeguards with regard to confidential files.
MARGINAL DUTIES & RESPONSIBILITIES
  • May be required to oversee and maintain the safety of the members, staff, and visitors. This includes the upkeep of fire and safety equipment.
  • Conducts all safety drills in conformance with current CARF and Agency standards and complete documentation as instructed.
  • May be required to work flexible schedules and be available to work on weekends or evenings.
REQUIREMENTS
  • High School diploma or GED
  • At least 21 years of age
  • Preferred: CRSS
  • Bachelor's degree in social work, counseling, psychology, or a closely related field
  • Any Bachelor's degree with 2+ years of experience in the field
  • 5+ years of experience in the field can be substituted for a degree
  • Trained in Basic First Aid and CPR
  • SHIFT: Monday - Friday 2:30 PM -11:00 PM

  • Competitive Pay: Pay Range: $23.10 hrly - 25.10 hrly ( Annually - $48,048 - $52,200 )
