Job summary

Qualifications

• Requires a bachelor’s degree in a related field; experience with youth preferred.
• Knowledge of mental illness (SED population) and crisis management.
• Ability to analyze problems, establish facts, draw conclusions, and make recommendations.
• Ability to build therapeutic relationships and maintain client confidentiality.

Responsibilities

• Provides Therapeutic Behavioral Services to an assigned caseload in accordance with state mandates and agency policies.
• Provides transportation to and from various locations in support of the program.
• Documents all services and maintains records in the electronic record.
• Participates in team meetings, supervision, in-service training, and conferences.
• Maintains client confidentiality and ensures release of information is handled per federal guidelines.

Position Summary:

The Case Manager provides direct support to youth and families through health interventions to assigned clients with severe and persistent mental illness who require individual community based rehabilitative services and/or group rehabilitative services to maximize the reduction of symptoms of mental illness in order to restore clients to the highest level of functioning.

Key Responsibilities & Role Highlights:

• Provides Therapeutic Behavioral Services to an assigned caseload of clients in accordance with the State mandates and established Agency and program policies, procedures, philosophy and client individual service plan.
• Provides transportation to and from various locations in support of the assigned program.
• Documents all services provided and maintains records utilizing the electronic record in accordance with agency policy/procedure
• Participates in team meetings, supervision, in-service training, and conferences according to established policies, procedures, and applicable regulations.
• Maintains client confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures.

Education & Experience Requirements:

• Requires a bachelor’s degree in related field. Experience with youth preferred.
• Must demonstrate knowledge of mental illness including the SED population, crisis management and the ability to establish and maintain therapeutic relationships with clients.
• Must demonstrate the ability to define problems, analyze data, establish facts, draw valid conclusions, and make recommendations.
• Must demonstrate positive attitude, commitment to provide superior services and enthusiastic support for the mission of the agency.
• Must establish and maintain effective working relationships.
• Must demonstrate the ability to represent the agency in a positive manner when in the community.
• Must be able to work with ill, disabled, emotionally upset and at times hostile individuals.
