Job summary

Unison Health is hiring full-time Care Coordinators to serve children and youth with behavioral or developmental health needs and their families. The role involves collaborating with families and community partners to connect youth to needed care and services.

Qualifications

• High School Diploma with 3 years’ experience or higher education with relevant experience is required.
• LSW/LPC licensure preferred.
• Experience in children’s behavioral health or related public sector human services is preferred.
• Proficiency with computer systems and software is expected.

Responsibilities

• Work directly with children, youth, and families in community-based settings in a hybrid remote/office and in-person model.
• Coordinate care services by scheduling appointments, managing referrals, and ensuring effective communication among clients, families, and providers.
• Maintain accurate client records in compliance with healthcare standards and regulations.
• Collaborate with internal team members and community partners to address client needs and support care continuity.
• Participate in training and professional development to enhance skills.

Education & Experience Requirements:

• High School Diploma with 3 years’ experience, OR
• Associate or Bachelor’s degree with 2 years’ experience, OR
• Master’s degree with 1 year experience
• LSW/LPC licensure preferred
• Experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers
• Expertise in one or more of the following areas: family systems, community systems/resources, case management, child and family counseling or therapy, child protection, or child development
• Proficient in computer systems and software
• Must possess a valid driver’s license, reliable transportation, and be insurable under the agency’s commercial policy; must carry personal auto insurance
