Job summary

Qualifications

• Minimum of a bachelor’s degree in Social Work, Counseling, Psychology, or a related field.
• Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed.
• Valid driver’s license and reliable transportation required.
• Experience working with children and families preferred.

Responsibilities

• Serve a limited number of youth and families at a time.
• Deliver intensive home-based case management services.
• Collaborate with clinical therapists to support individualized family service plans.
• Document all services provided and maintain accurate client records.

Position Summary

Unison Health is seeking a Case Manager to join our Home-Based Treatment (IHBT) team, providing services to youth and families with complex behavioral health needs. • Document all services provided and maintain accurate client records in the electronic health record system
• Ensure compliance with confidentiality guidelines and proper handling of release of information

Education & Experience Requirements

• Minimum of a bachelor’s degree in Social Work, Counseling, Psychology, or a related field
• Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed
• Valid driver’s license and reliable transportation required
• Must be insurable under the agency’s commercial liability policy
• Experience working with children and families preferred
• Must demonstrate knowledge in:
