The Fedcap Group is seeking a Case Manager in New York, NY to develop and monitor participant service plans, coordinate referrals, and support employment readiness. The role requires collaboration with internal teams to stabilize medical and mental health needs and remove barriers to employment.

Qualifications

• Bachelor's degree in social work or related field required.
• 1 year of case management or related experience preferred.
• Bilingual English/Spanish preferred.

Responsabilités

• Initiate services after clinical assessment or reassessment, orient participants to services, explain rights and responsibilities.
• Develop and monitor wellness plans for participants with untreated or unstable medical/mental health conditions.
• Meet regularly with participants to monitor progress and assist with barriers to employment (housing, DV, childcare, transportation, accommodations).
• Conduct regular phone follow-up to assess progress toward wellness plan goals.
• Outreach to re-engage participants who missed mandatory appointments or program requirements.
• Communicate with external medical and mental health providers to ensure compliance and timely completion of plans.
• Provide intensive individual coaching for participants in job search activities and work-related benefits issues.
• Case conference with other WeCARE providers (vocational evaluators, instructors, coordinators, counselors).
• Document participant services in Fedcap and HRA systems (SEAMS, FedcapCARES).
