The position offers a compensation of $23.08-$24.04/hr, and bilingual candidates are preferred.

Qualifications

• 1+ year of case management or related experience.
• Ability to develop and monitor comprehensive service plans.
• Experience with community-based medical and mental health providers.

Responsabilités

• Initiate services for participants following clinical assessments.
• Develop and monitor wellness plans for participants.
• Conduct regular follow-ups to assess progress on wellness plan goals.
• Assist participants with job readiness coaching and benefit issues.
