Job summary

Pierce County is seeking a bilingual Case Manager fluent in Samoan to manage caseloads of Medicaid clients. You will perform assessments, authorize services, oversee care plans, and monitor client progress using a laptop in clients’ homes.

This grant-funded role offers in-home support to older adults and people with disabilities while promoting safety and independence. The position is hybrid, with the first 3 months in-office followed by telework as probation concludes, providing meaningful

Qualifications

• Fluent in Samoan and English required for client communication.
• Experience in case management and coordinating services for older adults and disabled clients.

Responsibilities

This position is eligible for 10% field work specialty pay and 5% bilingual pay for hours worked.

Hybrid - The first 3 months in-office, then scheduled levels of telework until probation is complete.

Special Note:

This is a grant-funded position. Continued employment is contingent upon job performance/evaluation, continued funding of the program, and other appropriate County procedures.
