Job summary

Community Health Resources in Willimantic, CT is seeking a Case Manager for our 10-bed transitional living program for adults with Substance Use Disorders. This full-time role supports clients while they attend outpatient treatment and maintain discharge plans.

You will provide case management, assessments, crisis intervention, and collaboration with treatment teams, while documenting services and transporting clients as needed.

Qualifications

• Associate’s degree required; bachelor’s degree or equivalent experience.
• At least two years’ experience in a mental health setting preferred.
• Valid driver’s license.

Responsibilities

• Provides case management and coordinates with client supports and treatment teams.
• Conducts program-specific assessments and screens.
• Documents services and contacts in agency systems.
• Transfers clients as required by program.
• Participates in staff meetings, supervision and trainings.
• Obtains crisis intervention and de-escalation when needed.
