  • Help clients access resources including healthcare, education, employment, housing, transportation, public benefits, and other basic needs.
  • Connect clients with CIRI programs, community organizations, government agencies, and other service providers.
  • Help clients understand and navigate systems that can sometimes be complicated or overwhelming.
  • Follow up with clients and service providers to make sure referrals are moving forward and needs are being addressed.
  • Work closely with CIRI staff and community partners to coordinate services and resources.
  • Meet with clients in the Bridgeport office and at community and service locations as needed.
  • Participate in outreach activities and community events to connect individuals and families with available services.
  • Maintain accurate and timely case notes, service plans, referrals, documentation, and program data.
  • Provide transportation to clients when necessary and appropriate to help them access essential services.
  • Maintain confidentiality, professional boundaries, and a client-centered approach in all aspects of the work.
