Provides direct assistance to identified client including completing an assessment, person-centered service plan, and notes/documentation to facilitate the accomplishment of activities of daily living, personal service plan goals, encouraging community engagement, collaborating and advocating with outside organizations. Demonstrates and coaches the development of skills identified as goals in the service plan. Assist clients in the removal of barriers to self-sufficiency, including applying for and maintaining benefits. Provide transportation for client to maintain medical or other appointments outside the program. Complete all assigned documentation through communicating information to program staff or/and completing a progress note and uploading required documentation into client’s Electronic Medical Record and other human services system if required by position within 48 hours of service. Minimum client contact is every two weeks. Responds appropriately and in compliance with agency Policies and Procedures to emergent incidents/accidents that develop with client and/or in the environment of care by contacting and reporting to the chain of command and 9-1-1 or the Abuse Hotline, if appropriate.
Orient and facilitate the client with entry into housing/shelter. Maintain safety, security, and satisfaction of identified client by monitoring environment of care. Conduct a monthly housing check with emergency drill(s). Responsible for clearing out resident belongs after resident vacates property.
Meet with professionals from outside agencies to conduct outreach for various programs and collaborate for program success. This may include regular meetings with various groups. Conduct outreach in the community with the homeless population. Enter data into community data bases as appropriate and, if trained, complete SOAR applications.
Oversee a team of case managers in Lee County regarding the implementation and success of all the above areas. Provides ongoing supervision to program staff. Provide one-on‑one training to new staff and to assist in any problem area or procedural change for the team. Ensure coverage of case load, including documentation, for case managers during any sick, vacation, other leave, or when short staffed. Completes staff performance evaluations in coordination with Regional Manager according to CASL standards and implements agency personnel policies and procedures as necessary. Communicate regularly with supervisor regarding any ongoing staff issues. Monitors and manages staff scheduling and service provisions. Completes chart audits to assure the timely and accurate submission of service plans/service documentation, billing reports as required, and completion of monthly standards surveys. Assists in the interviewing and hiring of staff in conjunction with the Regional Manager and Director of Human Resources. Approve timecards for direct reports by deadline for payroll processing, including being responsive to fix errors and approve or deny PTO requests.
Responsible for assisting the Regional Manager with the day‑to‑day supervision, coordination, and implementation of team activities for the purposes of efficient and effective service delivery. Monitors the compiling, maintenance, and review of necessary client records to assure compliance with county and state regulations. Assists Regional Manager with the preparation and maintenance of statistics and reporting. Check company email while on duty to respond timely, appropriately, and professionally to communicate internally and externally.
Vast knowledge of theories, principles, and techniques related to services provided. General knowledge of administrative principles and procedures. Ability to establish/maintain effective working relationship with clients, associates, and agency personnel. Knowledge of community resources and ability to facilitate coordination activities. Ability to in the preparation of clear, concise reports and to communicate effectively, both orally and in writing. Ability to establish effective working relationships with clients and agency personnel. Follow and enforce company policy while displaying CASL Leadership Expectations. Attend monthly Health & Safety Committee meetings.
Qualifications/Education Requirements
Candidates must possess a Bachelor's Degree from an accredited university or college with a major in; counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services field and have a minimum of three years of full time or equivalent experience working with adults with severe and persistent mental illness,

Bachelor's Degree from an accredited university or college and five years, full time or equivalent, working with adults with persistent mental health diagnosis.
One-year supervisory experience is preferred.
Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population.
Must have knowledge of available resources for adults with severe and persistent mental illness.
Must have knowledge of the principles and practices of case management.
Demonstrates above average verbal and written skills.
Must demonstrate efficient computer skills (40 wpm).
Must demonstrate excellent of electronic health records and ability to effectively train.
Must demonstrate skilled knowledge and use of Microsoft Suite.
Must be able to work with diverse groups and individuals.

Must pass a drug screen and follow drug‑free workplace policy.
Valid FL driver’s license; must be able to drive with fewer than four combined violations and collisions in the past three years (excluding serious violations) in order to become and remain under the agency’s vehicle insurance policy.
Must maintain trainings as required.
Must have a negative TB screening within 30 days of hire and annually thereafter.

Management Expectations and Performance Standards
Case Manager Team Lead will maintain a caseload, provide and document a minimum of 30hours of billable services to clients per week, complete all opening paperwork for all new clients, review state outcomes quarterly, review FARS/FASAMS assessments and service plansat their due date or sooner if appropriate, complete initial leases and income certifications as well as their reviews annually, and complete all documentation on client services no later than 24hours after the service has been provided. Must comply with all company policies and procedures and all contract requirements.
Case Manager Team Lead will communicate regularly with supervisor to ensure expectations are met for funding, staffing, client issues, etc. Maintain appropriate boundaries in the workplace and work professionally (ethically), especially regarding confidentiality. Interact professionally with co‑workers, residents, and the community.
