Wisconsin Community Services, Inc. is seeking a Case Manager for the Targeted Case Management program in Milwaukee. You will build a therapeutic relationship, develop treatment plans, and coordinate care for adults with serious mental illness.

The role emphasizes psychosocial rehab, medication monitoring, and community resource collaboration while supporting clients toward housing stability and entitlement access. Strong independence and teamwork are valued.

Formación

• Bachelor’s Degree in a behavioral science required; MSW preferred.
• Master’s Degree encouraged; meet background check and driving record requirements.
• Must have a valid driver’s license and adequate automobile insurance.

Responsabilidades

• Develop a treatment relationship with consumers and formulate treatment plans.
• Provide psychosocial rehabilitation, symptom monitoring, and supportive counseling.
• Monitor client’s psychiatric medication regimen and physician-directed compliance.
• Coordinate with community resources and outside health care providers.
• Assist clients in housing, income resources, and access to services.
