Whole Health Clinical Group in Milwaukee, WI is seeking a Case Manager to provide care for adults with mental illness. You will develop and implement treatment plans and coordinate follow-up services.
Responsibilities include building treatment relationships, delivering psychosocial rehabilitation, monitoring medications, coordinating community resources, and assisting with housing and income supports.

Job Purpose: This is a professional position that is responsible for the care and treatment of mentally ill adults. This Case Manager will develop and implement the treatment plan, which will include the necessary follow-up, case management and intervention as required.

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)
Minimum Required Education: Bachelor’s degree in social work, Psychology, Community/Family Counseling
Minimum Required Experience: Minimum of 1 years’ working with adults with severe and persistent mental illness.

Issued By Governing Body (if applicable): Certification as a functional screener is not required but desired.

Physical Requirements: Work is performed largely in the community but may at times include an office environment or other various settings. This position frequently utilizes various office equipment including, but not limited to, a personal computer, multi-line telephone, fax machine, office copier. On occasion must be able to push/pull/lift/move objects weighing at least 20 pounds. Must have the ability to climb at least two flights of stairs. Must be able to safely push a person in a wheelchair on a level floor or on a wheelchair ramp in various weather conditions. Must be able to transfer a person from a wheelchair to the toilet, chair, or van seat/car with use of a gait belt and/or manual or power Hoyer lift. Occasional use of stairs.
Visual Acuity: The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.
Working Conditions: The hours of work will generally be during regular business hours and normally work 40 hours a week. This position requires participation in reoccurring ‘on-call’ duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekends. This position may work in a variety of indoor and outdoor work environments which may include, various outdoor weather conditions, loud noises, and fluctuating temperatures.
