WestCare in Fresno provides case management services to individuals enrolled in substance use disorder treatment. The role includes assessing needs, planning care, and linking clients to essential services while advocating for access to health care and community resources.

The position requires collaboration with a network of community partners, maintaining files, and adhering to confidentiality and regulatory requirements.

Competenze

• Two years case management experience preferred.
• Graduation from an accredited four-year college in a related field preferred.

Mansioni

• Assess and periodically assess client needs for case management services.
• Assist clients in transitions between levels of care.
• Develop and periodically revise the case management plan with service activities.
• Coordinate communication, referrals, linkage, and related activities.
• Monitor service delivery to ensure access to necessary services.
• Advocate for patients and coordinate linkage to physical/mental health care.
• Maintain client files per policy and contract.
• Develop knowledge of community resources and act as liaison.
• Maintain relationships with referring and supporting agencies and represent the agency at meetings.
• Must support WestCare Foundation mission and vision.
