Job summary

Qualifications

Responsibilities

• Assess and periodically assess client needs for case management services.
• Assist clients in transitions between levels of care.
• Develop and periodically revise the case management plan with service activities.
• Coordinate communication, referrals, linkage, and related activities.
• Monitor service delivery to ensure access to necessary services.
• Advocate for patients and coordinate linkage to physical/mental health care.
• Maintain client files per policy and contract.
• Develop knowledge of community resources and act as liaison.
• Maintain relationships with referring and supporting agencies and represent the agency at meetings.

POSITION SUMMARY:Person in this position will be responsible for providing case management services including assessment of service needs, care, service planning, linkage and advocacy for clients enrolled in SUD treatment services. Essential Job Functions include those listed below.ESSENTIAL JOB FUNCTIONS:Assessment and periodic assessment of individual needs for case management servicesAssist clients in transitions between levels of careDevelopment and periodic revision of case management plan that includes service activitiesCommunication, coordination, referral, linkage, and related activitiesMonitoring service delivery to ensure client access to necessary servicesPatient advocacy, linkages to physical and mental health care, transportation, and retention in primary careMaintain client files according to company policy and contract requirements.Develop and maintain working knowledge of available community resources and act as liaison.Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested.Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; andPerform all other duties as assigned.ESSENTIAL QUALIFICATIONS:To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. Employees that have requested a Medical or Religious Exemption, which has been approved by Human Resources will be required to test for the virus weekly.During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.CPR Certification, First Aid Certification, and an annual Tuberculosis test.EDUCATION and/or EXPERIENCE:Two (2) years case management experience preferredBasic computer skills.Good documentation skills.Be able to multi-task; andExcellent communication skills (written and oral).Graduation from an accredited four-year college with a degree in Psychology, Social Work, Rehabilitation Counseling, or a related field is preferred.High School Diploma required.LICENSURE/CERTIFICATION:Alcohol and Drug Registration and actively pursuing Certification, in good standing by an agency approved and recognized by the Department of Health Care Services (DHCS) is required at time of hire; Preference will be given to those candidates who are currently Certified.WORKING CONDITIONS:Work is primarily performed in an office or in a residential/outpatient treatment setting.Some outdoor activities may be required.UA testing of clients is required.Must be able to de-escalate situations in the program.
