Job summary

Beth Israel Lahey Health is seeking a Case Manager for the Student Assistance Program in Gloucester to provide school-based preventive services to youth at risk. You will work under supervision to assess needs, coordinate with school staff, and connect youth with community resources.

The role emphasizes prevention education, data collection, and outreach, with evening hours and local travel as needed. A Bachelor's in a related field is required along with experience with youth and substance use

Qualifications

Bachelor degree in counseling, psychology, social work, or related field.
Experience with youth, families and substance use issues; able to document electronically.
Valid driver's license with reliable vehicle and ability to travel/transport clients.
Ability to complete written documentation timely and accurately.

Responsibilities

Provide school-based preventive individual and/or group support under supervision.
Coordinate with school staff, parents, and community providers; assess needs.
Deliver prevention education through classroom presentations and events.
Plan and facilitate community-based activities; occasional evenings/weekends.
Maintain supervision, manage logs, submit paperwork and outcomes data.
Represent the program at events to advocate for youth substance abuse issues.
Perform other duties as assigned.
