Job summary

Universal Health Services in Bethany, OK, seeks a Case Manager/Step-Down Navigator to discuss stepdown services with eligible adults and coordinate discharge planning. You will work within Cedar Ridge at Bethany to streamline care transitions and communicate eligibility to leadership and departments.

In this role, you’ll collect and enter data in the Midas system for reporting, support referral development, and ensure compliance with ANA standards and facility policies.

Qualifications

  • Minimum one year of case management experience required, preferably in the mental health field.
  • Bachelor’s degree preferred and Case Management Certification preferred.
  • High school diploma or equivalent required; valid driver's license.

Responsibilities

  • Meet with eligible adult patients to discuss benefits of stepdown services as part of care.
  • Communicate eligibility to senior leadership and departments; collect data in Midas.
  • Coordinate between departments and assist discharge planning with therapists.
  • Develop referral sources and follow facility policies and ANA standards.

Responsibilities

The Case Manager/Step-Down Navigator is responsible for meeting with all eligible adult patients at Cedar Ridge at Bethany to discuss the benefits of stepdown services as part of their continuum of care treatment.

The Case Manager/Step-Down Navigator is responsible for meeting with all eligible adult patients at Cedar Ridge at Bethany to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership and associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes. The navigator will have a thorough understanding of the step-down process and will coordinate between departments. Assists therapists with discharge process, develops referral sources, and performs duties in accordance with the ANA Standards of Practice for Psychiatric Mental Health Care, and facility mission, values, policies, and behavioral standards.

Requirements
  • High school diploma or equivalent required
  • At least 21 years of age, required
  • Successfully complete a drug test
  • Successfully pass a national background check

Qualifications
  • Bachelor’s degree in related field preferred.
  • High school diploma or equivalent required
  • Minimum one year of case management experience required, preferably in the mental health field.
  • Thorough understanding of the inpatient workflow process and step-down process as it pertains to case management and discharge planning preferred.
  • Case Management Certification preferred.
  • Valid Drivers License, required
  • Must be computer proficient and have strong communications and analytical skills.
  • Exposure to psychiatric patients who may exhibit violent/aggressive behavior
