Job summary

Gándara Center in Springfield, MA is hiring a Case Manager for our Recovery Now program within the Behavioral Health & Recovery Services Division. This full-time role reports to the Program Director and emphasizes the Gandara philosophy and high commitment to the program.

The incumbent will perform assessment, service planning, and direct coordination of care for individuals served, focusing on OUD and social determinants of health. Bilingual candidates are encouraged to apply.

Qualifications

• Bachelor’s Degree in psychology or related field.
• Experience in child/adolescent development, behavioral treatment, substance use/dual recovery issues, and family treatment/engagement/education regarding psychiatric and substance abuse recovery issues.
• HIPAA and privacy laws and regulations knowledge in mental health.

Responsibilities

• Provide assessment and service planning to address OUD and identified social determinants of health.
• Develop, implement, and monitor service plans aligned with identified needs.
• Communicate effectively with persons served and collaterals in a timely manner.
• Attend external and internal meetings relevant to individuals served and services.
• Ensure progress notes and service plans are completed professionally and documented in the EHR in a timely manner.
• Provide wraparound care and act as primary contact for individuals served.
• Attend trainings as required by Program Director.

Job Summary:

We're in search of a case manager to work on a full time basis for our Recovery Now program, part of our Behavioral Health & Recovery Services Division. The case manager is accountable to the Program Director, who is responsible for providing supervision and training. The case manager is one who actively promotes the philosophy of the Gándara Center and maintains a high level of commitment to the program.

Duties and Responsibilities:

• Provide assessment and service planning to address OUD and identified social determinates of health
• Develop, implement, and monitor service plan consistent with identified needs
• Communicate effective and in a timely manner with persons served and collaterals
• Attend external and internal meetings relevant to individual served and/or services
• Ensure that persons served progress notes, service plans, and other required documentation is completed professionally and accurately and in the electronic health record in a timely manner
• Ensure that consultation on the person’s served behalf is conducted
• Provide wraparound care and serve as primary point of contact for individual served
• Attend relevant trainings as required by Program Director

Minimum Qualifications:

• Bachelor’s Degree in psychology or related field
• Specialized training/experience in child/adolescent development, behavioral treatment, substance use/dual recovery issues, and family treatment/engagement/education regarding psychiatric and substance abuse recover issues
• Must understand the population and information sensitivity (i.e. HIPAA and privacy laws and regulation) protocols required in mental health
