Qualifications

• Bachelor’s degree in Social Work required.
• Licensed BSW or LMSW within 6 months of hire or transfer.
• Preferred: 2+ years of social work experience.
• Experience in acute care hospital case management is a plus.
• Certification in case management (ACM or CCM) preferred.

Responsibilities

• Assess and coordinate patient care across the continuum using guidelines.
• Support patients and families with discharge planning and coping strategies.
• Advocate for patients and coordinate with healthcare teams for care quality.
• Ensure compliant and ethical practices per federal/state hospital policies.
• Assist transitions to next level of care and manage readmission risks.
• Identify community resources and present post-acute provider options.
• Work primarily in the Emergency Department to support care needs.
