Responsabilités

• Provide case management and discharge planning services in a timely and cost-effective manner.
• Complete psychosocial assessment of identified patients and plan care.
• Coordinate with physicians, nursing, case managers and allied health professionals to achieve outcomes.
• Assist in education, referrals and advocacy to expedite timely discharge.
• Ensure adherence to hospital policies and patient safety protocols.
