Job summary

Catholic Charities, Diocese of Trenton is seeking a dedicated SCMS/SOARS case manager to design, document, and monitor individual program plans for clients. You will assess needs, determine eligibility, counsel families, and maintain required records.

The role collaborates with senior case managers, networks with community resources, may provide counseling and transport clients to appointments, and handles additional tasks under supervision of program leadership.

• Counsels and aids individuals and families requiring agency assistance.
• Maintains client records and completes required documentation according to agency and accreditation standards.
• Collects documents to prepare clients for applying SSD/SSDI.
• Collaborates with Senior case managers in outreach to engage clients in need of this service.
• Networks with other providers to generate community resources for participants.
• May provide supportive counseling in individual and group settings.
• May transport consumers to appointments.
• All other assigned tasks under the lead of SCMS / SOARS Senior case managers or the Program Director.

ESSENTIAL FUNCTIONS: Provide SOARS and SCMS case management services, including design, documentation, and monitoring of individual program plans for clients.

• Assesses client needs; investigate their eligibility for services.
• Counsels and aids individuals and families requiring agency assistance.
• Maintains client records and completes required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs.
• Collects all pertinent documents to prepare clients for applying SSD/SSDI.
• Collaborates with Senior case managers in outreach initiative to engage clients in need of this service.
• Networks with other providers to generate community resources for participants.
• May provide supportive counseling in individual and group settings.
• May transport consumers to appointments, etc.
• All other assigned tasks under the lead of SCMS / SOARS Senior case managers or the Program Director.
