Job summary

Porchlight Inc. in Madison, WI seeks a Case Manager to provide housing-focused support to guests of the Porchlight Men's Shelter, enroll and orient new guests, and help identify goals to pursue permanent housing.

You will maintain regular contact, develop housing plans, coordinate services, and document progress. Strong communication, organization, and professionalism are essential; you will work with guests facing homelessness and related barriers. Shift hours: Saturday to Wednesday, 1pm–9pm.

Job description

The Case Manager provides housing-focused case management and individualized support to guests of the Porchlight Men's Shelter. This position enrolls and orients new shelter guests, and helps guests identify goals, address barriers, connect with community resources, and pursue permanent housing.

Case Managers are responsible for maintaining regular contact with assigned guests, developing and updating housing plans, coordinating services, and completing accurate and timely documentation. The position requires strong communication, organization, professionalism, and the ability to work effectively with individuals experiencing homelessness, mental illness, substance use disorders, medical concerns, and other barriers to housing stability.

Shift Hours Saturday through Wednesday, 1pm-9pm
