Job summary

Lantern Community Services Inc is looking for experienced Case Managers to work at their new women's shelter in the Bronx, New York. This full-time role entails conducting assessments, linking clients to services, and developing service plans to support independent living and housing goals.

The successful candidates will possess strong relationship-building skills, experience with homelessness and mental health challenges, and must be available for flexible shifts, including evenings and weekends.

An hourly wage of $27.47 to $35.71 is offered.

Qualifications

• Minimum of two years of social services experience required.
• Experience working with individuals experiencing homelessness and/or mental health challenges.
• Valid Food Handler’s License or ability to obtain within six months of hire.

Responsibilities

• Conduct intake and assessments to develop individualized service plans.
• Coordinate services with Housing Specialist and external partners.
• Document all case management activities.
