Job summary

Job description

JOB SUMMARY:
Responsible for coordinating and overseeing the care and services provided to individuals served by MCS,
ensuring that their needs are met in a timely, efficient, and compassionate manner.

JOB SPECIFIC RESPONSIBILITIES:
  • Maintains quality of programming through adherence to agency standards, DDS regulations and Human Rights standards, Disabled Person Protection Commission’s (DPPC) Mandated Reporter Guidelines, DPH/MAP regulations, and OQE License/Certification indicators.
  • Conducts assessment(s) based on information gathered from individual served, family members, DDS and other relevant stakeholders for treatment planning and/or ISP development.
  • Develops goals based on individual needs and challenges identified through assessments to be implemented in ISP or Treatment Plan, attends annual ISP meeting, and monitors the implementation of goals by caregivers and DSPs.
  • Completes semi-annual progress summaries.
  • Reports in HCSIS as required.
  • Coordinates services and resources around medical, housing, mental health, and financial needs.
  • Serves as a connection for individuals to access services and beneficial, quality community experiences.
  • Maintains accurate, thorough, and up to date documentation.
  • Provides emotional support and guidance to individuals, their families, caregivers and/or DSPs.
  • Assists individuals to make informed decisions.
  • Works collaboratively with other stakeholders including DDS, doctors, therapists, social workers, family members, etc. and attend meetings and appointments as necessary.
  • Recruits, interviews, and hires DSPs. Provides program orientation, training, and ongoing supervision, completes performance evaluations and disciplinary action, if necessary, to DSPs and Care Providers.
  • Ensures required training needs are met and plans and protocols are followed.
  • Oversees and maintains quality of services and licensing requirements in homes.
  • Follows program’s emergency response policy and protocols.
  • Participates with the program team in reviewing new referrals.
  • Supports continuity of care by assisting with essential service delivery, including providing direct and/or personal care support when necessary to ensure adequate staffing and the well-being of individuals served.
  • Maintains a flexible, yet reasonable, work schedule to meet the needs of the program.
  • Performs any other duties as assigned by supervisor.
JOB COMPETENCIES REQUIRED:

Case Managers, Care Managers, and Site Managers must have strong communication, interpersonal,
organizational and time-management skills; the ability to identify and understand individuals’ needs;
knowledge of community resources and services; empathy and problem-solving skills; working knowledge of
documentation and reporting procedures; strong computer skills.

  • Contributes to a positive work environment.
  • Commits to following all agency rules and regulations as outlined in the Employee Handbook.
  • Trained in and upholds the principles of Positive Behavior Support (PBS).
  • Complies with all responsibilities as a mandated reporter.
Required Qualifications, Skills, and Training
