Job summary

Service Coordination, Inc. is seeking Service Coordinators to provide case management for individuals with intellectual and developmental disabilities, building positive relationships with people and their support teams.

Position focuses on developing and monitoring Person-Centered Plans (PCPs), advocating for services, and coordinating resources to meet state-mandated standards. Priorexperience with developmental disabilities is preferred.

Qualifications

  • Bachelor’s degree in a human services field, or a non-human services degree with two years of human services experience.
  • Experience working with people with developmental disabilities is preferred.

Responsibilities

  • Advocate for individuals, provide resource information, and educate them to increase self-determination.
  • Educate individuals and families on coordinating and advocating for services.
  • Encourage empowerment and independence; teach principles of self-determination.
  • Collect information for DDA eligibility, determine priority, and develop PCPs.
  • Plan, facilitate, and update PCPs; document service needs and outcomes.
  • Coordinate resources and services to achieve PCP outcomes; share information with team members.
  • Monitor PCPs; conduct on-site visits, track progress, and submit required reports (e.g., RECON, AIR).
  • Adhere to SCI policies, HIPAA, and confidentiality; collaborate with colleagues and participate in meetings and trainings.
  • Meet performance benchmarks and maintain HIPAA records securely; use tracking/time management tools.
  • Demonstrate customer focus, interpersonal savvy, integrity, and adaptability; uphold organizational values.

Physical Demands

Standard office environment. Ability to operate a computer and mobile devices. Ability to travel and visit individuals in various settings. May require lifting 10–25 pounds periodically.
